Pathways version control
Review Dates
- To establish a core group comprising of senior experienced clinical experts from across Wessex, and will include Salisbury, Dorchester, Poole, Bournemouth, Hampshire, Southampton, Portsmouth and the Isle of Wight
- To review existing pathways and update where necessary
- To provide governance and sign off for intrapartum care pathways
- To share learning from excellence, good practice and good projects
- To provide a platform for open and honest discussion of difficult cases
- To provide support and sign off for Wessex PEGS Emergency Proformas
- To identify areas of quality improvement that align with MatNeoSIP and support system level improvement
- Provide a platform to discuss latest national guidelines, impact and issues of implementation
- Feed into wider National work programmes i.e. WACP, LMS, Clinical Networks (inc Maternal Medicine), ODN
Membership of the Group
- Membership of the Group will include at least one clinical representative from each of the following Maternity Services
- University Hospital Southampton NHS Foundation Trust
- Hampshire Hospitals NHS Foundation Trust
- Isle of Wight NHS Trust
- Portsmouth Hospitals NHS Trust
- Dorset County Hospital NHS Foundation Trust
- University Hospital Dorset NHS Foundation Trust (formerly Royal Bournemouth and Christchurch Hospitals NHS Foundation Trust and Poole Hospital NHS Foundation Trust)
- Salisbury NHS Foundation Trust
- Wessex Patient Safety Collaborative
- These representatives will be able to facilitate decisions on their organisation’s behalf.
- Members can bring with them additional representatives of their trust for learning i.e. junior doctors/ trainee midwifes but they will not be responsible for facilitating decisions on behalf of their organisation.
Sign off process and dissemination and implementation
- The pathways will be developed, and changes agreed where necessary and signed off by the group.
- Each Trust representative will be responsible for presenting the final updated pathways to their own clinical governance boards for information and dissemination
- The final updated pathways will be re issued to all trusts in PDF format
- The review group will meet every four months to review relevant pathways that require review and agree any changes.
- Implementation of the pathways can, where appropriate, will be supported in the MatNeoSIP Patient Safety Network using quality improvement methodology.
- The multidisciplinary group that developed this guidance have made considerable efforts to ensure the information upon which these guidelines are based is accurate and up to date. Revision may be deemed appropriate if and when new data or national/ international guidance becomes available. These guidelines do not replace the need for application of clinical judgment by clinicians to each individual presentation and specifics of the situation.
Chair
- Chair will be responsible for ensuring the meetings are held in a timely manner
- Will ensure that each review follows the agreed format
Administration and Support
Administrative support will initially be provided by Wessex Patient Safety Collaborative
- To include minute taking, action tracking and the distribution of papers.
- Updating the pathways, document control and governance
- The copy of the pathways will be held and stored by Wessex Patient Safety Collaborative on their central drive.
Pathways version control
- All pathways will be dated and subject to version control to ensure that changes and amendments can be tracked and documents are current
Review Dates
- Terms of Reference and membership to be reviewed annually
